A council and the Care Quality Commission (CQC) have initiated inquiries following an incident where a 3-year-old boy was injured in a crocodile enclosure.

A new report by the CQC revealed that some healthcare professionals, including midwives, were subjected to extended periods without rest, often working double shifts with no sleep.
